Community Teams

  • Hospital at Home – service providing urgent nursing care for patients in a home environment, including nursing and residential homes.
  • Hospice in your Home – providing care and support for end of life (palliative) patients in their own home.
  • Specialist Palliative Care Team – clinic-based service at Wrightington Wigan Leigh NHS Foundation Trust.
  • Wigan & Leigh Hospice Kildare Street Hindley, WN2 3HZ – 01942 525566
  • Community Matrons – provide co-ordination of care for individuals with complex or long-term conditions.
  • Think Wellbeing – NHS therapy for adults age 16 and over with common mental health problems like mild to moderate depression, anxiety and stress. This service is run by North West Boroughs Healthcare NHS Foundation Trust and you will need to be registered with a Wigan GP.

Pharmacy Team

  • Stock piling of medication
  • If the patient is unsure of what medication they take
  • If the patient is unsure why they take their medication
  • Compliance issues including how to use inhalers etc.
  • Understanding medication changes from hospital discharge or new medication
  • Understanding reducing or increasing dose plans
  • Swallowing difficulties
  • Advice and Guidance with the Administration of covert medication and documentation
  • Lining medication up

Community Link Workers

Care Coordinators

  • Prepare for follow-up clinical conversations with primary care professionals
  • Be actively involved in managing their case and make choices that are right for them
  • Develop personalised care and support plans
  • Understand and manage their conditions, ensuring changing needs are addressed
  • Provide coordination and access to other appropriate services and support
  • Tend to work with people with multiple appointments, frail/elderly and people with long-term conditions

Healthy Routes Advisors

  • Stop smoking and harm reduction behaviour change support, including access to Nicotine Replacement Therapy (NRT)
  • Healthy Weight Management support including single point of access
  • Mental wellbeing support using a self-care approach implementing Connect 5 and 5 ways to wellbeing principles
  • Develop knowledge, skills and confidence to become more active
  • Support offered in reduction of alcohol, building confidence to access to other appropriate services

Frailty and Care Homes

  • Provision of the weekly Home Round for all 4 care Homes attached to the PCN
  • Development of the Personalised Care & Support Plan for each permanent resident within the 4 care homes.
  • Providing Advanced Practice clinical Intervention for any acute needs of residents when attending the individual homes.
  • Provides support, advice, guidance and education to care home staff in order to enhance quality of care delivery and patient  experience.
  • Works with the Multidisciplinary Team including :primary, community and acute NHS services, and local authorities, to ensure appropriate input into the MDTs working with care home secured and consistent (to include dentist, optometrist, social workers as required).
