Provision of the weekly Home Round for all 4 care Homes attached to the PCN
Development of the Personalised Care & Support Plan for each permanent resident within the 4 care homes.
Providing Advanced Practice clinical Intervention for any acute needs of residents when attending the individual homes.
Provides support, advice, guidance and education to care home staff in order to enhance quality of care delivery and patient experience.
Works with the Multidisciplinary Team including :primary, community and acute NHS services, and local authorities, to ensure appropriate input into the MDTs working with care home secured and consistent (to include dentist, optometrist, social workers as required).
