Travel Health Questionnaire
To help us offer the appropriate advice, please fill out the online form before coming to see the nurse. Then hand in the completed form to reception. You will then be told when to contact us back to make an appointment to see the nurse for any vaccinations you will need. The nurse will need to check the completed form before an appointment is made.
Please contact the surgery in advance as the vaccines should be given at lease 6 weeks before travel.
